Duels TO THE DEATH - in arena

Ever wanted to fight someone till one of you was really dead,
not that foofy pretend death you get in arena, while still
being sure that griefers, gankers, lolzpkers, teamers, and
other assorted scum couldn't interfere?

Here's your chance!

The new addition is CHALLENGE person TO THE DEATH.

This is a variant on CHALLENGE. It only works for duels at
present. It works in all arenas at present. It cannot be
disabled in any arenas, at present.

The general idea is that if your opponent accepts, it's
just you two, mano-a-mano, power to power, skill vs. skill.
and nobody else, in an arena all for yourselves.

If you don't time out, then someone will die, and the one
who dies loses experience. The one who killed the other
person gains experience.

Herbs, salves, elixirs, smokes, tattoos, and the like are
all used up, just like you weren't in the arena.

When there was a killer, then the bodily remains (if any)
will be in the possession of the killer at the end.

Everybody will still be cleared out of arena at the end,
and the dead will still be dead. Might want to make
arrangements to get resurrected!

A few idiosyncracies remain - things that may exist or
be handled differently in arena (heartstones, totems,
envenoms) will still be handled differently, at least
till we figure a straightforward way to change that.

Those who leave arena after a duel to the death won't
have arena grace.

I'm sure there are a few oddities left about it. Let us
know, via a bug report, or let me know privately via a
message, and we'll get it all straightened out.
