Apostasy changes

We have introduced a couple of changes to the Apostate class today, the
details are as follows:
- BLOODWORM disruption no longer stacks.
- BLOODWORM attack speed has been decreased.
- NIGHTMARE no longer gives a hellsight-based affliction on each attack,
just the hellsight affliction itself.
